- This is an unofficial guide. TDLR does not publish it. The form and the TDLR website are the authority.
Each quote below is from the form. The line number after a quote, for example "page 1, line 1.12", is the line in the transcription at guides/source/lic003.md. In the quotes, this guide removes the extra spaces that the PDF text extraction adds inside some words.
What the form does
You use this form to ask the TDLR Executive Director to renew a license that expired too long ago to renew online. You send the form by mail with the late renewal fee and all the other renewal requirements. The form does not replace those requirements. For an electrician, the TDLR renewal page also asks for the license application of your license type. For a journeyman electrician, that is the Journeyman Electrician License Application (ELC-LIC-005).
Who must use this form
Use this form only if your license expired at least 18 months ago and less than 3 years ago.
"YOUR LICENSE MUST BE EXPIRED FOR AT LEAST 18 MONTHS BUT LESS THAN THREE YEARS TO MAKE THIS REQUEST." (page 1, lines 1.2 to 1.3)
- Expired less than 18 months: do not use this form. Renew through the normal renewal process. For electricians, TDLR renews these online.
- Expired 3 years or more: do not use this form. You cannot renew. You must apply for a new license.
"LICENSES EXPIRED LESS THAN 18 MONTHS SHOULD FOLLOW THE RENEWAL PROCESS. LICENSES EXPIRED MORE THAN THREE YEARS CANNOT BE RENEWED AND YOU MUST APPLY FOR A NEW LICENSE." (page 1, lines 1.6 to 1.7)
The form also accepts a written request in place of the form (page 1, line 1.3).
Before you start
Complete every renewal requirement before you mail the form. For an electrician, this includes the continuing education hours. A journeyman electrician needs 4 hours of TDLR approved continuing education.
"All renewal requirements must be met prior to the issuance of the renewal of your license. This includes continuing education, renewal courses, reports, and any additional renewal requirements needed." (page 1, lines 1.40 to 1.42)
All the fields are on page 2 of the form. Page 1 contains the instructions only.
The fields, in form order
Three fields are for you to complete by hand on the printed form: item 2 (Social Security Number), the signature, and the date. This guide gives no content for them, other than what the form says.
1. Licensee Full Name, or Company Name
Write your full legal name in this order: last name, first name, middle name, suffix. Write it exactly as it is on your expired license. A suffix is Jr., Sr., II or III. "Mr." is not a suffix. A company writes the company name in the "Company Name" field.
"Provide your full legal name in the spaces provided. (Last, First, Middle Name, Suffix) Examples of a suffix include Jr., Sr., and II. (Mr. is not a suffix.) or write the COMPANY NAME. This should be exactly as it appears on your expired license." (page 1, lines 1.8 to 1.10)
If your name changed, attach a document that shows the new name. The form gives three examples: a Social Security card, a Driver License, or a marriage license.
"If a name change has occurred, supporting documentation is required." (page 1, line 1.10)
2. Social Security Number or Federal Tax ID Number (complete by hand)
Complete this field by hand. A person writes the Social Security Number. A company writes the Federal Tax ID Number. Texas law requires the number for a license.
"Social Security Number disclosure is required by Section 231.302c(1) of the Texas Family Code in order to obtain a license. Your Social Security Number is subject to disclosure to an agency authorized to assist in the collection of child support payments." (page 1, lines 1.13 to 1.15)
3. License Type
Write the name of your license type, for example "Journeyman Electrician" or "Master Electrician".
"(Example: Journeyman Electrician, Air Conditioning Contractor, Manicurist, Tow Operator, etc.)" (page 1, line 1.17)
4. Expired License Number
Write the license number that TDLR gave you. It is on your expired license. You can also find it with the TDLR license search.
"The license number issued to you by TDLR when you originally submitted your application." (page 1, lines 1.19 to 1.20)
5. License Expiration Date
Write the date your license expired. The time from this date to today must be 18 months to less than 3 years.
"The date the license was no longer valid." (page 1, line 1.21)
6. Mailing Address
Write your current mailing address. TDLR sends your mail to this address. A post office box is allowed.
"Provide your current mailing address. This is the address where we will send you mail. This address can be a post office box." (page 1, lines 1.22 to 1.23)
The form asks for the address in this order: P.O. Box or number, street name, suite or apartment number, city, state, ZIP code (page 2, line 2.13). You can add the 4 extra ZIP code digits (page 1, lines 1.23 to 1.24).
7. Email Address
This field is optional. Write your email address only if you agree that TDLR sends license messages and required notices to you by email.
"Provide your email address only if you agree to the following statement: By providing my email address I authorize the Texas Department of Licensing and Regulation (TDLR) to send licensing communications and required notices to me by electronic mail." (page 1, lines 1.26 to 1.28)
If you write an email address, you must tell TDLR when it changes. If you do not, you will not get the notices (page 1, lines 1.28 to 1.29).
8. Phone Number
Write a phone number with the area code. Use a number where a person answers during the day. An office number where TDLR can leave a message is allowed.
"Provide a telephone number, including the area code, where we can reach you during the day. This may be your office phone number where we can leave a message." (page 1, lines 1.31 to 1.32)
9. Criminal history
Mark "Yes" or "No". The question asks about the time since TDLR issued your most recent license.
"Since the most recent license was issued, have you or the controlling person been convicted of, or placed on deferred adjudication for any felony or misdemeanor other than a minor traffic violation?" (page 2, lines 2.16 to 2.17)
If you mark "Yes", complete one Criminal History Questionnaire (TDLR form LIC002) for each offense and attach it.
"If YES, complete and attach a Criminal History Questionnaire (PDF) for each offense." (page 1, lines 1.34 to 1.35)
10. Reason you did not renew
Mark one reason why you did not renew before the license expired. The form gives these choices:
- Death or serious illness in the family
- Out of country
- Job loss
- Birth or adoption of a child
- Other (write the reason on the line beside "Other:")
"Choose the reason why you did not renew your license before it expired. You may use the back of the page or an additional page if necessary." (page 1, lines 1.36 to 1.37)
11. Statement of Applicant, signature and date (complete by hand)
Read the statement. Then sign and write the date by hand.
"Carefully read the statement of applicant before dating and signing your application." (page 1, line 1.39)
In the statement, you agree to obey the laws and rules of your occupation. You also state that your information is true. False information can cause TDLR to deny the request or revoke the license (page 2, lines 2.26 to 2.28).
- Signature of Applicant: sign by hand.
- Date Signed: write the date by hand.
Where to mail the form
Mail the completed form, the fee and the required documents to:
Texas Department of Licensing and Regulation P.O. Box 12157 Austin, TX 78711-2157
"SEND YOUR COMPLETED APPLICATION AND REQUIRED DOCUMENTS TO: Texas Department of Licensing and Regulation P.O. Box 12157 Austin, TX 78711-2157" (page 1, lines 1.45 to 1.48)
TDLR does not return the documents you send. Keep a copy of the form, every attachment, and your check or money order. Do not send cash (page 1, lines 1.49 to 1.50).
The fee
The late renewal fee for this form is two times the renewal fee.
"The late renewal fee is twice the amount of the renewal fee. Refer to the website for program rules and renewal fee amounts." (page 2, lines 2.4 to 2.5)
Example: the journeyman electrician renewal fee is $30, so the late renewal fee for this form is $60. Source: the TDLR journeyman electrician renewal page, https://www.tdlr.texas.gov/electricians/renew/individuals/journeyman-electrician.htm, read 13 Aug 2026. Check the TDLR renewal page of your license type for the current amount before you mail the form.
Send the fee with the form.
"ALL FEES ARE NON-REFUNDABLE AND THE FEE MUST ACCOMPANY THIS APPLICATION" (page 2, line 2.2)
